Dear Edwin,
I have created a menu for frontend slave site creation with single slave site creation menu type.
Although I have disabled showing "site prefix" option, it is showing up in the frontend
Of course cache is off (anyway the menu was newly created).

Verify the "layout" that you have selected.
If you have created your own front-end layout, perhaps you didn't tested if the field is hidden or displayed.
To know which layout is seleted, return in the "mneu item" and this is the first parameter that define the layout (template).

Thank you, that was the problem.
There were two default option: 'default' and '<Default>'. The second was the right option.
If I may add, I couldn't answer so far as you forum was unavailable because of database connection error, I am telling it as perhaps you don't know about it, and it was not the first time.
