I have linked to the front end form to create a new site. When I fill int he fields and click Save, it takes me back to this same page, removes the fields I entered, and shows no success or error message. The site is not created.

Also, I dont know if its related, but the Administrator username field is missing. Just nothing there. The Administrator username field is then grey. So I set the Admin user in the template and hid these fields from displaying. (but the issue existed befofe I did this also).

Perhaps you could try to follow the video 14 or 13 that show how to create a website from the front-end.

Enter exactly the same value.
In most case, we are using keyword to be generic.
